A Home Care Package is a Federal Government support package which aims to enable people to stay longer in their homes as they age. Home Care Packages are generally managed by Home Care Providers who assist clients to manage their Home Care Plans.

If you have a Home Care Plan and have been recommended for a formulated meal replacement or a protein supplement by a dietitian, doctor or another approved health professional then you are eligible for subsidy. You need to discuss this with your Home Care Provider. Optimal Essentials Complete Nutrition complies for the requirments as it is a formulated meal replacement. Protein Powder for Seniors also complies as it is a formulated supplementary food. Both comply with the FSANZ Standard 2.9.3 which is a requirment of the Home Care Plan guidelines.

Other Optimal Essentials products are not considered formulated supplementary foods under the FSANZ standard and therefore are not available for subsidy through a Home Care Plan.

If you order Complete Nutrition or Protein Powder for Seniors thoguht wour Home Care System we will invoice your Home Care Provider direct and once they approve your order we will have the product delivered directly to your home. You do not have to pay any money at the time of order for these products.

If you decide to order additonal products which have been recommended for you they are not eligible under the Home Care Package and will have to be paid at the time of order.

One completion of the Questionnaire the recommendation from the our dietitian will immediately come up on your screen and an email will be sent to you and your Home Care Provider.

To review your Questionnaire results, orders and communication with us you need to login into the system by clicking on the Dashboard menu item. You will then need to put in your login email address and password.

The questionnaire has been developed based on the Mini Nutrition Assessment tool "MNA®".The MNA® is an internationally recognised, validated nutrition screening and assessment tool that can identify patients aged 65 and above who are malnourished or at risk of malnutrition. The MNA® was developed nearly 20 years ago and is the most well validated nutrition screening tool for the elderly. Our dietitians use this tool to assess your nutrition status.

Yes the Nutrition Assesement Questionnaire is available for everyone over the age of 65. It has been specifically been designed for Seniors. You do not need to be on a Home Care Package.

Yes you can setup a login for a family member or carer when you first complete the Questionnaire which will allow them to receive copies of your nutrition assesment and any orders you make. They can even make orders on your behalf if you would like. 

You are able to order your products on a subscription so you make sure you never forget to order your products and are left short. The subscription can be cancelled or modified at any time.
